Ursinus Hosts Passport Day

Want to apply for a U.S. Passport? Passport Day at Ursinus is Wednesday, Oct. 26, in Olin Hall, room 104. Drop-in hours are 10 a.m. to 4 p.m.  All Ursinus students, faculty, staff, their families and the local community are welcome.

The website link to the forms is: http://www.travel.state.gov/passport/forms/forms_847.html. Applicants can fill out the form online or print out a blank form and fill it out.

Checks should be made payable to the “Department of State”. Reminder that personal checks have to show a preprinted name and address to be acceptable.

 To apply, this is what is needed:

 1. Form DS-11, if you are applying for the first time or under age 16.  All DS-11 applicants must appear in person.

Form DS-82, if you can submit the previous U.S. passport issued to you in the last 15 years, when you were age 16 or older.

 2. U.S. Citizenship Evidence (Original)

 3. Valid Photo ID

 Previously issued, undamaged U.S. passport

 Naturalization Certificate
 Valid Driver’s License
 Current Government ID (city, state or federal)
 Current Military ID (military and dependents)

4. Photocopy of Valid Photo ID on plain white, 8 1/2 x 11″ standard paper stock

Submit a clear photocopy of the front and back of the valid photo ID that is being presented at time of passport application.

5. One Color Passport Photo (Size: 2”x2”)

 6. Appropriate Passport Fees – Money order and checks only. Personal checks must show a preprinted name and address.

Cost for First-time Applicants:

  •  Adults: $135 (age 16 and older)
  •  Minors: $105 (under age 16)

Cost for Renewal (by mail):

  • Eligible Adults: $110

 U.S. Passport Card  – only acceptable for land and sea travel

Cost for First-time Applicants:

  • Adults: $55 (age 16 and older)
  • Minors: $40 (under age 16)

Expedited service is available for an additional $60.

Processing time for expedited service is approximately two weeks.

Processing time for routine service is four weeks.

 For minor children under 16, if both parents are listed on the birth certificate:

1)    Both parents have to apply together with the minor to sign the passport application or,

2)    A notarized consent form, DS-3053, with a photocopy of the non-appearing parents’ ID may be submitted in lieu of the non-appearing parent, or

3)    An original court order granting sole legal custody to the applying parent may be submitted.
